BANKS AND TRUSTS

AmericaUnited Bank and Trust Company USA, Schaumburg – consent order to stop engaging in unsafe and unsound banking practices.

The Elgin State Bank, Elgin – consent order to stop engaging in unsafe and unsound banking practices.

BARBER, COSMETOLOGY, ESTHETICS AND NAIL TECHNOLOGY

Lisa Logan, Rockford – cosmetologist license (011-235819) reprimanded and fined $300 due to intermittent practice on a lapsed license.

Michelle Salerno, Crystal Lake – cosmetologist license (011-229753) placed in refuse to renew status due to material misstatement, willfully making false records in connection with
cosmetology practice, and unprofessional conduct.

Demetrius Curry, Rockford – (unlicensed) ordered to cease and desist the unlicensed practice as a barber.

Anthony Hunt, Rockford – (unlicensed) ordered to cease and desist the unlicensed practice as a barber.

DETECTIVE, ALARM, SECURITY, FINGERPRINT VENDOR AND LOCKSMITH

Wallace Sanders, Rochelle – permanent employee registration card (129-311840) revoked due to his felony conviction for aggravated battery and his failure to report that
conviction to the Department.

MEDICAL

Victor Colin, Elgin – physician and surgeon license (036-107075) reprimanded and fined $500 for violation of Medical Practice Act related to his treatment of a patient in his private
practice.

Shashi Patel, Dixon – physician and surgeon license (036-054292) reprimanded and fined $500 for failure to properly monitor a patient during the administration of anesthesia in a
hip replacement surgery.

NURSING

Sheilah Dye, Rockford – registered nurse license (041-311188) placed on indefinite probation after defaulted on an Illinois educational loan and has now entered into a repayment agreement.

Michelle Turner, Machesney Park – licensed practical nurse license (043-066058) restored to indefinite probation with work restrictions effective upon payment of fees and filing of forms.

OPTOMETRY

Ziva Optical, Schaumburg – (unlicensed) assessed a $50,000 civil penalty due to unlicensed practice of optometry by soliciting patients for screening procedures which may not have been medically necessary.

REAL ESTATE

Marta Njos, Poplar Grove – real estate broker license (475-110108) revoked due to settlement paid out of Real Estate Recovery Fund on behalf of respondent.

Good move by the local government taking this very seriously. Every professional should have the correct licensing in able to perform their jobs. One good example of this was an incident whereas the barber on our neighborhood did not took his Cosmetology License Illinois and there was a grieve malpractice that happened only to find out that the barber faked his license. Kudos to the government a move so that we can avoid those kind of incidents.
