This is a multi-faceted problem that began in 2010, with the departure of two village employees.
Administrative Assistant Maureen Brzinski said she filled out an LGHP cancellation form, so the former employees would be removed from coverage, and forwarded it to the company.
According to Village Attorney Roxanne Sosnowski a representative of LGHP, Diann Reed, said the forms had not been forwarded to the company.
As such the company continued to bill Poplar Grove for the full premiums. The village, in turn, reduced the amount owed for the two departed employees and over a period of time the shortfall increased to $17,366.
